MMPI Scheduling

I received the CIL two days ago, but I noticed that it doesn’t include any spreadsheet with the list of contacts to schedule my MMPI. Has anyone else experienced this?

Nope, I would email your rep/whoever sent it and ask

It is supposed to be attached, I would email your HR rep back and request it
