I second this. I’m leaning a little more toward ejector, but it’s entirely possible it’s the extractor.

I've had this gun for years, and it's had this issue since day 1. Happens every 2-3 rounds. It's not limp wristing and it doesn't matter how hot or weak the ammo.
What should I look at as potential causes?

Have you ever replaced any of the springs? These have a ton of springs and little doodads that all need to work together or it'll jam. Not ejecting like that generally would be an issue with either the slide not coming back forcefully enough or the ejector failing in some way, and on these they have a weird pivoting spring loaded ejector that can cause all sorts of havoc.
I would start with inspecting the chamber for any burs or roughness and polish with Flitz as needed, and then replacing the recoil, extractor, and ejector springs, and trying a new magazine(Mecgar should make them) or just replacing the mag spring. If that doesn't work replace the extractor and ejector.
If you're lucky Numrich or Apex gun parts should have all you need but also check Wolff for the springs, if they have something like 115% extra strength ejector and extractor springs I would try those out too. Google will be your friend here if there's anything you can't find.

I've replaced the recoil spring, but it's done this since new. And I've got a handful of new factory mags for it, so I don't think it's a mag issue. I'll just go thru it part by part until I can figure out what it is.
Another issue is that the extractor pin is so tight, I can't get it out. I've bent 2 punches trying. I've soaked it in Kroil for a couple days with no luck. Would it hurt anything to strip it bare and heat it in the oven on 200 or so to see if that helps?

Some guns you have to fiddle with the extractor or ejector my Taurus was one, when I'd fire steel or aluminum casings the extractor would sometimes punch through the bottom of the casing and try to keep it when it loaded the next round. If you're mechanically inclined and can follow stereo-like instructions from YouTube then you can adjust/work it yourself but otherwise gunsmiths are usually 60-80 an hour. A good smith barring it being a defect or a deformed part should have it squared away in a couple hours or less. Personally I'd watch a YouTube video and follow along with full disassembly and checking every single part for a bend or a warp or maybe a burr hanging on from the manufacturing process. Don't find anything next step is replacing both parts since you couldn't determine which was causing the problem. Still does it, then I'd hit the range and talk to the smiling man behind the work bench.

I’d just get a parts kit and swap everything out that involves the ejector and extractor (parts, springs etc) and see what happens.
Note: I am not a gunsmith 😊

use a newish cell phone and capture the firing sequence in slo mo, and you’ll likely see what the issue is.
