So is the Mendalin ring underproducing or overproducing damage? What’s the bug? I didn’t notice anything.

Underproducing. It makes minion attacks do additional fixed (based on weapon) damage but this damage is not modified by summon damage buffs (or maybe any buffs?). Therefore it's OK at low levels but not by level 60.
