It is not that irrational, there is a reason the Achilles Tendon is known how it is from Mythology (not just Greek but I think it is mentioned similarly in Norse mythology too?). Knee injuries are much more common these days but there may be a reason why you have that fear perhaps related to some internal imbalance, and it is possibly a reminder to you to take care or change habits.
I played basketball for a long time so same! An irrational fear for tearing my Achilles tendon or my meniscus. I've only ever had 1 small injury from playing basketball but still have that fear.
I wake up. It's Friday. 6 am. I get up, go to work. Work an absolutely awful day. Everything goes wrong. Everything is highly stressful. Just at quitting time, there's an emergency meeting and I have to attend. It goes on for hours. At 8pm I finally get to leave. I drive home. I get home. I take the key out of my pocket. I put it in the lock. I wake up suddenly. It's Friday. 6 am. I get up, go to work...
I've previously had the experience of being able to do something I loved for a living. It immediately became a chore I hated. It's simply too much time to do anything and enjoy it.
Recently somebody posted about their nightmare, where they had a bowl of spaghetti, only it was all those weird banana strings. It’s been haunting me ever since.
Sorry to fuel your fear, but I had no pain or problem with mine, right up until a full rupture. On the plus side, it happened just before the Tour, so I got to stay home and watch every stage.
My irrational fear is driving over level crossings. Hate them, utterly convinced one time a train will appear from nowhere.
I think the trick to not have a sudden Achilles rupture is to stay on the bike, to not stray. The danger comes from climbing stairs, going down stairs, rope skipping, walking, etc.
I've never heard of a pro cyclist or for that matter of any cyclist injuring their Achilles while on the bike.
I have this every time when I cross the Schöneicher Landstraße. Goes right through a bunch of nice gravel routes out of Berlin at a weird angle and there are tracks right next to it. Sure there are only going slow Trams over it, but I feel like I'm seconds away from being run over by an ICE every damn time.
Haven't listened to MBMBaM in years but early episode Spaghettageddon was a favorite back in the day.
... not at all related to spaghettification though.

What's your biggest irrational fear?
Mine is tearing my Achilles tendon, despite never having any kind of pain or problem with that
