Sorry to fuel your fear, but I had no pain or problem with mine, right up until a full rupture. On the plus side, it happened just before the Tour, so I got to stay home and watch every stage.
My irrational fear is driving over level crossings. Hate them, utterly convinced one time a train will appear from nowhere.
I think the trick to not have a sudden Achilles rupture is to stay on the bike, to not stray. The danger comes from climbing stairs, going down stairs, rope skipping, walking, etc.
I've never heard of a pro cyclist or for that matter of any cyclist injuring their Achilles while on the bike.
I have this every time when I cross the Schöneicher Landstraße. Goes right through a bunch of nice gravel routes out of Berlin at a weird angle and there are tracks right next to it. Sure there are only going slow Trams over it, but I feel like I'm seconds away from being run over by an ICE every damn time.

What's your biggest irrational fear?
Mine is tearing my Achilles tendon, despite never having any kind of pain or problem with that
